Make a Claim
  1. Download and print out the .PDF of the claim form. Download Claim Form Here

  2. Fill out the top portion (Part 1) of the form. Please make sure you provide all required information and print clearly.

  3. Bring the claim form to your veterinarian to fill in Part 2 of the form and sign.
    Note: Ask your veterinarian if they are using EVE (Online insurance claims filing for veterinary hospitals and clinics.)

  4. Sign and date your claim form before you fax or mail it in.

I was pleased to learn that PetCare would cover exactly what I had expected for the costs of treatment for Alexandra's kidney problems.
